How much generally is the employer portion of Plan Cost? If the employer portion of the cost is 2.5 - 3.5% of payroll, employer and employee both are comfortable

Can the “couple” rate apply to a parent with 1 eligible dependent? Yes. It can apply to a married couple, common-law spouses, or a single parent with 1 eligible dependent.
